Appointments

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

We accept most major insurance plans

No-fault insurance

No-Fault Insurance is state-mandated coverage in New York. It’s deemed “no-fault” because it is made available to you regardless of who caused, or was at fault in the accident in which you were injured. It is designed to pay for medically necessary treatments that you may need as a result of an injury sustained in an automobile accident, including orthopedic care.

In New York State, motor vehicle operators, pedestrians, and cyclists are eligible for no-fault coverage. It is important to note that motorcyclists do not qualify.

No-fault insurance typically covers a majority of car-accident related fees and expenses. There are limits in place, but the state of New York requires a minimum of $50,000 of no-fault insurance coverage.

Fees & payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made.

We accept cash, checks, MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.

Please request all prescriptions and authorizations during our regular office hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.

Medical forms

There are various forms on our website. Please download the forms as directed by your doctor and/or staff. Fill out the forms and take them with you during your scheduled visit. This will help expedite the registration process.
